Check-in documents

Mandatory documents required to enter the sanatorium:

For guests over 14 years of age:

• Identity document
• Sanatorium-resort card with obligatory indication of fluorographic research (it is obligatory, in accordance with the Order of the Ministry of Health of the Russian Federation dated 15.12.2014. N 834n “On approval of unified forms of medical documentation used in medical organizations providing medical care on an outpatient basis, and procedures for filling them out,” accounting form 072/y).
• Certificate or mark in the package of documents on the absence of contact with patients with COVID-19 within the previous 14 days, issued by a medical organization no later than 3 days before departure.
• Compulsory or voluntary health insurance policy (for residents of the Russian Federation).
• Power of attorney from the organization to receive original contracts (when issuing a voucher through companies paying for their employees).
• Copies of payment orders with a bank mark (in case of non-cash payment).
• Exchange voucher or voucher (when issuing a voucher from travel agencies).

For guests under 14 years of age:

• Birth certificate.
• Sanatorium-resort card for children without fluorography (it is mandatory, in accordance with the Order of the Ministry of Health of the Russian Federation dated 15.12.2014. N 834n “On the approval of unified forms of medical documentation used in medical organizations providing medical care on an outpatient basis, and the procedures for filling them out,” accounting form 076/y).
• Certificate from a pediatrician or an epidemiologist about the absence of contact with patients with infectious diseases.
• Certificate of results of parasitological examination for enterobiosis (necessary for visiting the pool).
• Certificate or mark in the package of documents on the absence of contact with patients with COVID-19 within the previous 14 days, issued by a medical organization no later than 3 days before departure.
• Compulsory or voluntary health insurance policy (for residents of the Russian Federation).
